What are Neuromodulators (also known as neurotoxins)?

Fine lines and wrinkles form in the face due to a loss of tissue volume, a decrease in subcutaneous fats and lipids, and diminishing collagen and elastin. The result is thinner, drier skin that is less able to bounce back when facial muscles contract. Repeated muscle activity creates a groove that eventually forms into a wrinkle when less elastic skin has trouble rebounding from movement.

Injections made from botulinum toxin are neuromodulators that work by reducing the muscle activity that causes and exacerbates wrinkles. Produced by naturally occurring bacteria, botulinum temporarily paralyzes facial muscles by disrupting the signals that allow neurons to communicate properly. It is used for medical and cosmetic treatments but is perhaps best known for reducing the appearance of fine lines and wrinkles. Products that contain the botulinum toxin are best known by their brand names: Botox, Dysport and Xeomin.

  • Botox – FDA-approved to treat wrinkles since 2002, Botox has long been the gold-standard of cosmetic botulinum use. Botox is more concentrated than alternatives such as Dysport and can be used to treat a wider variety of wrinkles. Botox may also be more effective at treating deeper lines and smaller areas. The onset of Botox is 24 to 48 hours, but significant improvements may take up to a week to appear. Botox results last an average of four months, but this varies by patient.
  • Dysport – Dysport is formulated a bit differently than Botox or Xeomin. Dysport has smaller molecules, allowing it to diffuse and create a softer, feathering effect that appears more natural. This may also make it more suitable for treating larger areas, in which case fewer injections would be needed. Dysport reportedly has the quickest onset, with noticeable results appearing two to three days after treatment and lasting for about four months.
  • Xeomin – Xeomin uses a purified form of botulinum toxin, meaning it contains no accessory proteins or additives. The additives in Botox and Dysport may prompt an immune response in some people that decreases effectiveness with continual use. The absence of carrier proteins in Xeomin also reduces the risk of an allergic reaction. Xeomin is often used for patients who have developed a resistance to botulinum toxin and no longer respond to other treatments. Xeomin has the slowest onset, with results developing after four days and maximum effect appearing after about 30 days. Xeomin results last for three to six months.

Ultimately, Botox, Dysport and Xeomin are more similar than different, but some patients and providers find that they prefer one brand over another for a variety of reasons. How do Botox, Dysport and Xeomin differ from dermal fillers?

Dermal fillers are injectables that restore lost facial volume beneath the skin to reduce wrinkles and other signs of aging. Many brands use hyaluronic acid, but others are made of calcium, collagen and synthetic-based ingredients. Unlike botulinum, fillers do not affect muscle activity. Dermal fillers can treat lines that are present when your face is at rest (known as static wrinkles), while botulinum toxin-based injectables target dynamic wrinkles (those that result from facial expressions).

What to expect during your treatment

The sensation is often reported as a brief pinching or pulling at the injection site, followed by a feeling of tightness.

What to expect after your treatment

Botox, Dysport and Xeomin require no downtime or recovery, although you may be discouraged from laying down or rubbing the area for a short period after the procedure. You may notice some bruising or redness at the injection site, but this should quickly subside. Depending on which formula was used, results will begin to develop over the next few days.
